NEW STUDENT REGISTRATION

New Student Registration For All Grades Entering Smithfield Public Schools:

New student registrations are held by appointment only at the Smithfield School Department Administration Office, 49 Farnum Pike, Smithfield, RI 02917.

All registration forms must be completed prior to your appointment. For your convenience, these forms are available below as PDF fillable documents that can be completed and printed at home. These forms are also available at the Administration Office and can be picked up at any time. Please contact the Administration Office at 401-231-6606, Ext. 510 to schedule an appointment once all forms are complete.

Please be sure to have all items on the CHECKLIST FOR CENTRAL REGISTRATION with you at the time of your appointment.

Kindergarten registrations for the following school year take place in January, by appointment only, at the elementary school in the district where the student resides. Children entering Kindergarten may be screened by calling Child Outreach at 401-231-6608.
